Atostek’s Solution Enhances Preventive Care – Pilot in Pirkanmaa Could Benefit up to One Million Finns

Atostek is delivering a technological solution to the Wellbeing Services County of Pirkanmaa to support preventive healthcare by identifying high-risk patients who should be invited for treatment. The pilot project could benefit up to one million Finns and significantly free up resources in the social and healthcare sectors.

A pilot project funded by Sitra has been launched in the Wellbeing Services County of Pirkanmaa, aiming to develop proactive social and healthcare service models. Atostek is providing the technical solution for the project, enabling high-risk patients to be identified and invited for preventive care.

The solution utilizes patients’ medical histories and the Duodecim Terveyshyöty-tool.

“Our general practitioners and nurses can use this tool to identify high-risk patients in the population, detect care gaps, and shift the focus of care from corrective actions to prevention,” says Riku Metsälä, project manager in Pirkanmaa.

The health and care planning tool developed in collaboration with Pirkanmaa has already received positive feedback from patients, particularly for its transparency in the Kanta services.

The goal is for the technical solution and new operational model to be completed by the end of this year, after which they can be expanded to other wellbeing services counties. Finland has approximately one million people with multiple chronic conditions, whose continuity of care and preventive measures could significantly improve well-being and reduce the need for services.

“With the ERA service we’ve built, patient data and comprehensive medical histories can be securely and efficiently made available to healthcare professionals. This allows professionals to focus on patient interaction and treatment implementation, without patients having to repeat the same information over and over,” says Matti Eskola, Atostek’s Head of Healthcare Innovation.

The service requires patient data sharing permissions through Kanta. The screening of patient data and identification of patients who would benefit most from care is always conducted anonymously. Only the treating physician has access to personal data.
